Welcoming your child into the Church
To begin, please contact the Parish Office. We’re happy to walk you through the process.
The questionnaire
Parents and godparents must fill out the Baptismal Questionnaire before attending the preparation class. Download the forms below, or stop by the office to pick them up.
Return completed forms by email to general@olacc.org or in person at the Parish Office. The form must be signed in the presence of a priest — please bring it to the office during regular hours.
What to bring along
- A copy of the child’s birth certificate
- If a godparent is registered at another parish, a letter of eligibility from that parish
What happens next
Once a priest has reviewed the questionnaire, we’ll reach out to schedule the Baptismal Preparation Class. The baptism is scheduled after all paperwork is in and the class has been attended.

Qué sigue
Una vez que un sacerdote haya revisado el cuestionario, le contactaremos para programar la clase de preparación. El bautismo se programa después de recibir toda la documentación y asistir a la clase.

Anointing of the Sick
Christ’s comfort in serious illness
For a family member who is seriously ill, hospitalized, facing surgery, or nearing the end of life — please call the Parish Office and a priest will come. Don’t wait until the last moments; the sacrament is for healing as well as for the dying.
